On June 10, 2015, a Dynamex Courier’s automobile was stolen while transporting bags containing inter-office mail and U.S. Mail items from RIA. When the automobile was recovered, all RIA items except U.S. Mail items were located. Upon notification of the incident, RIA and Invision Sally Jobe immediately investigated. Less than two hundred pieces of mail were involved, and some of those items included patient personal health information (such as patient name, date of birth, ordering requisitions, imaging studies and/or imaging reports).
After diligent investigation, we identified the intended recipients of most of the mail but cannot definitively state that every such individual was identified. Unless a patient had evaluation or imaging at one of our facilities between June 1 and June 9, 2015, or had requested reports or information during that time frame, there is little likelihood of involvement.
